If the levator ani muscles (aka pelvic floor muscles) are very tense they can refer to the tip of the penis by compressing the dorsal nerve to the penis, a branch of the pudendal nerve. As well, because the pudendal nerves (right and left) emerge along the inner pelvic wall to the perineum, there can be compression at the area just inside the sitting bones, which is a place of attachment of various muscles/fascia, including that of the gluteus and superficial pelvic floor muscles.
So relaxing the pelvic floor, and massaging the area near the sitting bones with a tennis or lacrosse ball might be worth a try.

I also suffer from something like this, a pain in the left side of the tip. I have mild, intermittent pain, sometimes it comes as severe pain without warning and leaves a sensation. I could describe the latter as having a prick or pinch inside the urethra on the left side. I already went to the urologist several times, he recommended some anti-inflammatories, which helped to a certain extent, but I still have this pain for 4 years. It’s annoying and makes me feel vulnerable, like I can’t let my guard down. Well, it is a quite sensitive area. I tried massaging the area with suitable objects from inside the urethra, it was somewhat uncomfortable at first, but it is the only way I have found to find a little relief.

I’m sorry to hear that you have experiencing this for so long. Did anyone you saw consider nerve compression? This could happen at the pudendal/dorsal nerve to the penis, which can be compressed at the pelvic floor, or sitting bone area. Some clues to this may be having to strain for bowel movements, a sense of pelvic floor tension, or changes to erections. Nerve compression can also occur at the left genitofemoral nerve which can be compressed by the abdominal muscles or in the spermatic cord.
